Medical InsuranceMedical Insurance Forms COBRA Continuation Coverage Rights The College offers eligible employees two types of health insurance plans through MVP Health Care: a Preferred Provider Organization (PPO) with a national provider network and a Health Maintenance Organization/Point of Service (HMO/POS) with a regional network of providers. A Preferred Provider Organization (PPO) provides financial incentives to utilize participating providers (in-network benefits), but does not require a primary care physician or referral process although some services require pre-authorization. A PPO also provides benefits for non-participating providers' services (out-of-network) subject to a deductible and co-insurance. Under a Health Maintenance Organization/Point of Service (HMO/POS), plan benefits are covered when delivered, arranged or authorized by a Personal or Primary Care Physician. Services provided by non-participating physicians are not covered unless determined to be medically necessary and arranged by a participating physician and approved by the Plan's Medical Director. The HMO Plan includes a POS rider for limited out-of-system benefits. Employees may waive coverage through the College and receive an opt-out credit equivalent to $1,000 a year for full-time employees, provided they can produce evidence of coverage under another health insurance plan. The College's contribution to health insurance coverage is based on one's salary and the coverage selected. Plan benefit summaries and contribution rates are available by selecting the appropriate links above. Enrollment forms and lists of participating providers are available from Human Resources. Participating providers may also be located through MVP's website. Select "Doctor Search" and then the type of plan (PPO or HMO/POS). Individuals may elect a change in their health insurance coverage during the College's open enrollment period in November of each year. Changes will become effective on January 1.
