2008 Open Enrollment


Welcome to Hamilton's Annual Benefits Open Enrollment for 2008. During this annual open enrollment period, which is Monday, November 5 through Friday, November 16, you have the opportunity to review and change your benefits to meet your needs for the upcoming year. Any changes you request to your enrollments will be effective January 1, 2008.

The following list was developed to make it easier for you to know what action(s) you need to take when reviewing the enclosed information. When you have read through each bulleted item below and completed any necessary forms, then you are done and need to take no further action; it's that easy!
  • I read the Benefits Guidebook and reviewed the Decision Guide on Page 15.
  • I read the 2008 Benefits Information.
  • Health, Dental, Vision – complete a form only if you desire a change to your current elections OR if you are not enrolled and want to be enrolled. I want to enroll or change my election and have completed the forms, OR I have checked this box because I am not enrolled and don't want to enroll, OR I am already enrolled and don't want to make changes.
  • FSA (Flexible Spending Accounts) – If you want to participate for 2008, you must complete an enrollment form.  I have completed a form, OR I have checked this box because I do not want to participate in 2008.
Please keep in mind that after the conclusion of this open enrollment period you may not change your insurance and FSA elections unless you have a qualifying event and the change you are requesting is consistent with the event. Qualifying events include a change in marital status, dependents, residence, or your or your spouse's employment status. If one of these events happens, you must notify Human Resources and complete applicable forms within 30 days of the event or you will not be able to make the change until the next open enrollment period.

The employee handbooks are being revised; therefore, some of the terminology in these materials may differ from what is in the handbook.
