Hamilton College requires a completed health history and an immunization record to be submitted upon admission to the College.
Continuing care is contingent upon receipt of this completed information.
Class registration cannot be completed until immunization records (in compliance with New York State Law and CDC guidelines) and required health form documents are on file with the Health Center.
All immunization and COVID vaccine/booster Exemption Request Forms must be completed annually and submitted by July 1 for the upcoming academic year, and December 14 for January admissions.
