Medical Records Requests
The Health Center retains immunization and medical records for seven years after graduating from the college.
If you would like to request copies of your medical and or immunization records, please submit a written request with your name, graduation year, what records you are requesting (please be specific), include a fax number or a mailing address and to whose attention these records will be sent to. Your signature is required at the end of your request.
Records less than five pages can be faxed, anything over five pages must be mailed.
